What Is Superbill Therapy and How Does It Work?

You found a therapist you connect with. They do not accept your insurance. You assume that means you are paying entirely out of pocket with no reimbursement. That assumption is often wrong. Superbill therapy gives you a path to use your out-of-network insurance benefits and recover a portion of your therapy costs. Here is exactly how it works.

What Is a Superbill?

A superbill is a detailed receipt that your therapist provides after a session. It contains specific information that your insurance company needs to process an out-of-network reimbursement claim. A standard superbill includes your therapist’s name, license type, and National Provider Identifier (NPI) number, the date of your session, the diagnosis code (ICD-10), the procedure code (CPT code) that describes the type of service provided, the fee you paid, and your personal information including name and date of birth.

This document is not the same as a regular receipt. A regular receipt confirms payment. A superbill provides the clinical and billing information your insurance company requires to evaluate your reimbursement claim.

What Is Out-of-Network Therapy?

Therapists fall into two categories for insurance purposes. In-network therapists have a contract with your insurance company. They bill your insurer directly and charge you only your copay or coinsurance amount. Out-of-network therapists do not have that contract. They set their own rates and collect payment directly from you at the time of service.

Many of the most experienced therapists in New York City practice out-of-network. This gives them more control over their caseload, session length, and treatment approach. It also means their services are not subject to insurance company limitations on session frequency or treatment duration.

How Does Superbill Reimbursement Work?

The process has four basic steps.

First, you pay your therapist directly at the time of your session. Most out-of-network therapists collect payment by credit card, check, or bank transfer at the end of each appointment.

Second, your therapist provides you with a superbill. Some practices send superbills monthly. Others provide them after each session. Ask your therapist upfront how often they issue superbills and in what format.

Third, you submit the superbill to your insurance company. Most insurers allow you to submit claims through their online member portal, by mail, or through a third-party reimbursement app. You attach the superbill as documentation and complete a short claim form.

Fourth, your insurance company reviews the claim and sends you a reimbursement check or direct deposit for the covered portion of the session fee. The amount you receive depends on your specific plan.

How Much Will Insurance Reimburse?

Reimbursement amounts vary by insurance plan. Most plans with out-of-network benefits cover a percentage of what they define as the “usual and customary rate” for therapy in your geographic area. This rate is set by the insurer and may be lower than your therapist’s actual fee.

A plan might cover 70% of the usual and customary rate after your out-of-network deductible is met. If your therapist charges $250 per session and your insurer’s usual and customary rate is $180, you would receive 70% of $180, which equals $126 per session after your deductible is satisfied.

Your out-of-network deductible is separate from your in-network deductible. It is typically higher and must be met before reimbursement begins. Once you meet it, reimbursement applies to all subsequent out-of-network claims for the rest of your plan year.

How to Check Your Out-of-Network Benefits

Call the member services number on the back of your insurance card before starting therapy. Ask these specific questions. Does my plan include out-of-network mental health benefits? What is my out-of-network deductible? What percentage does my plan reimburse after the deductible? What is the usual and customary rate for outpatient psychotherapy in New York City? Is there a limit on the number of sessions covered per year?

Write down the answers and the name of the representative you spoke with. Insurance companies can give inconsistent information, and having a record protects you if a claim is disputed later.

Why Many New York City Therapists Work Out-of-Network

Insurance-based therapy comes with constraints. Insurers require therapists to submit a diagnosis code to approve sessions, which becomes part of your permanent health record. They can limit session frequency and require justification for continued treatment. They also set reimbursement rates that many experienced therapists in New York City find unsustainable.

Out-of-network practice removes those restrictions. Therapists can focus entirely on your care without administrative interference. For clients, this often means access to more experienced providers, longer sessions, and treatment that is not cut short by insurance approvals.

Is Superbill Therapy Right for You?

Superbill therapy works best for people who have out-of-network mental health benefits in their insurance plan, can afford to pay the session fee upfront and wait for reimbursement, and want access to a specific therapist who does not accept insurance.

It is less practical for people with no out-of-network benefits, very high out-of-network deductibles that are unlikely to be met, or limited cash flow that makes upfront payment difficult.
